Why Do Telehealth Startups Fail? The Five Killers We See Repeatedly

1. Vendor sprawl
The founder assembles a web designer, a platform vendor, a provider group, a pharmacy contact, and a marketing agency, none of whom have met. Every handoff is a seam, every seam leaks time, and six months later the founder is a full-time project manager for a business that has not launched. The single most common killer.
2. The ad account ban
Health is a restricted advertising category. Brands that launch without LegitScript certification, or run creative with prohibited claims, get their ad accounts restricted, sometimes before the first campaign matures. No ads, no patients, no business. Certification and compliant creative are not optional extras. They are the license to acquire customers.
3. The payment processor shutdown
Telehealth is high-risk in payment underwriting terms. Founders who plug in a generic processor discover this when payouts freeze mid-month, with revenue locked in limbo. Processing built for telehealth, underwritten correctly from day one, is boring exactly the way you want it to be.
4. Retention neglect
Telehealth economics are subscription economics. A brand that acquires patients but has no follow-up care, no labs, no messaging, no reason to stay, pays acquisition costs over and over for revenue that never compounds. The unglamorous machinery of month four is where the business actually lives.
5. The launch-only partner
Plenty of vendors can get a site live. The brands that die at month three had nobody to call when the pharmacy question, the state rule, the chargeback, or the A2P rejection showed up. Before you sign with anyone, ask one question: who do I call when something breaks after launch, and is that person a builder or a salesperson?
The pattern
Every killer on this list is infrastructure, not ideas. Founders bring vision and drive. What kills them is plumbing. Fix the plumbing before it exists and the failure modes above simply never fire.
